Form 10-K405 was not a different report. It was a regular annual report on Form 10-K where the company checked the cover box saying that disclosure of delinquent Section 16 filers under Item 405 of Regulation S-K was not contained in the report. EDGAR gave those filings their own form type, so in the 1990s a large share of annual reports were filed as 10-K405 rather than 10-K.
The SEC eliminated the checkbox distinction in 2002, and EDGAR stopped accepting 10-K405 after early 2002. Any study of annual reports from 1994 to 2002 has to include 10-K405 alongside 10-K, or it will silently miss many companies and years.
